Open Access Program

How It Works

1. Referral or Self-Request – Your primary care provider may refer you, or you can request an open access appointment yourself.
2. Quick Questionnaire – Complete a short health assessment online to confirm eligibility.
   Select “Schedule Your Colonoscopy” below.
3. No Office Visit – If you qualify, you will bypass the initial consultation and schedule the
   procedure directly.
4. Scheduling & Preparation – The practice will provide clear instructions for prep and
   coordinate your colonoscopy date.

*If you have certain health conditions and/or take certain medication you may not qualify for Open Access Colonoscopy Program. You will be notified to schedule an office appointment prior to the procedure to discuss the best screening options for you.
